Unique Disability ID (UDID) Card

Description :

The UDID card shall bring a host of benefits to the Persons with Disabilities. The UDID card will be the single document of identification, verification of the disabled for availing various benefits in future.

Eligibility:
Physically or mentally disabled persons above the percentage mentioned:

• Mentally handicapped - 35%
• Orthopedically - 40%.
• Deaf & Dumb - 90 db & 100 db.
• Blindness - 90% or above.

Documents Required:

• Scanned Copy of Aadhar Card /Driving License /Pan Card /Ration Card .
• Application form
• Address proof
• Bank passbook
• Identity document:Voter ID card, PAN, Aadhar card
• Recent colour photo.
• Disability Certificate.
• Scanned image of signature (Optional)
• Scanned copy of Disability Certificate (Only for those Persons with Disability who have been issued disability Certificate by the competent Authority)
